There are lessons to be learned in Steve's story. It highlights the importance of early detection of elevated or rising PSA level, making accurate diagnosis which can be at times confusing like in his case and choosing the optimal treatment that will most likely result in a cure based on published data. Looking back, at age 55, radical prostatectomy instead of focal cryotherapy might have been a better choice and will more likely result in a cure. Prostate cancer is the most common cancer in men and 250,000 Americans will face the challenge of new prostate cancer diagnosis every year. Steve's story is important to share.

Arming My Body, Mind and Spirit For Battle
I am going to make some changes and adjustment  to maximize my chances  of winning and living healthier longer. I need other people to help me with this challenge. This includes healthier diet, stronger body, restful sleep, positive attitude, relaxation and meditation. 

I am planning to use continuous glucose monitoring with Libre 3 Plus to accurately determine the effect of what I eat on my blood glucose in real time. Takes the guessing out of it. 


Proton Therapy with Pencil Beam Scanning
There are several effective treatment options with very similar 10 - year survival rate - radical prostatectomy, photon (X-ray) radiation and accelerated Proton Beam therapy. I chose Proton Beam Therapy - greater precision with reduced harm to surrounding normal tissues and fewer long term side effects. (This modern machine is like a simplified miniature version of the $10B Large Hadron Collider - the largest, most complex machine ever built by man, at CERN in Geneva that finally discovered the elusive Higgs boson in 2012!) 100% Medicare covered. There are only about 40 proton treatment centers in the US and three of them are in New Jersey where I live. I chose Penn Medicine - Virtua Proton Treatment Center in Voorhees where I received exceptional care from Dr. Graeme Robert Williams during my initial consultation. Ask your doctor about it.

My radiation treatment simulation was completed on Wednesday, April 2, 2025. My radiation dose prescription is 70 Gray Units delivered in 28  fractions - the total absorbed radiation dose delivered in 28 days, Monday through Friday. My first treatment is on Wednesday April 16, 2025. My graduation day is on Friday, May 23, 2025.


ArteraAI Prostate Test - Answers an Important Question About Androgen Deprivation Therapy
Helps determine whether androgen deprivation therapy (ADT) will be beneficial or not by using AI to  identify the presence of specific patterns in the digital images of the biopsy slides. This helps avoid unnecessary exposure to hormone treatment when it is not likely to be helpful. My result was negative biomarker - no benefit with ADT. Ask your doctor about it.


PSMA PET-CT Scan - The Staging
Is the cancer still localized - just confined within the prostate or has it spread? The test uses latest machine that combines PET and CT scans in one system and can even detect micro metastasis. Treatment options may change based on the result of this test. This is a 2-3 hour test. 100% Medicare covered. Mine was negative. This is better than CT scan and bone scan in detecting spread outside the prostate. Ask your doctor about it.


Decipher Genomic Classifier Test 
Does the main treatment chosen, in my case - Proton Therapy, need further finetuning? Decipher test helps to determine the likelihood of the tumor having a more aggressive behavior based on the biopsy samples already obtained. My result was "high risk". It is useful in further adjusting the treatment in order to have best possible long term clinical outcomes based on the current data available. 100% Medicare covered. Ask your doctor about it.


MRI-Ultrasound Fusion Targeted Needle Biopsy 
The next step is to confirm if I have prostate cancer or not, but I don't want the usual random 12-core needle biopsy at the urologist's office that most men get. Is there something better? Yes, there is - a previous MRI image is combined with real-time ultrasound of the prostate using a specialized machine and the biopsy needle is guided to the mass where the cancer might be hiding. And it was diagnostic - Gleason 3+4 = 7. Looking back, if I had known this more advanced approach is available, I would not have waited 7 years to have the biopsy. 100% Medicare covered. Unfortunate, most urologists are not trained to perform this specialized biopsy. Dr. Jamison Jaffe performed my biopsy at the Princeton outpatient surgical center under general anesthesia and he was excellent. Ask your doctor about it.


3T Multiparametric MRI with and without Gadolinium
Once I decided to take the next step, I scheduled a 3T MP MRI of the prostate to see if there is a suspicious mass hiding in my prostate. And there it was - a PI-RADS 5 mass which is cancer until proven otherwise. With this information, there is a clear target to biopsy. No blind random hit or miss needle biopsy for me. 100% Medicare covered. Ask your doctor about it.

Why Proton Beam Treatment?
Since choosing the type of treatment has the greatest long term impact on both quality of life and longevity, I need to have a better understanding of the fundamental differences between proton therapy and X-ray (photon) therapy so I can have more confidence that I am making the right choice.

Choosing between proton beam therapy and X-ray (photon) therapy for prostate cancer involves understanding nuanced biological and quantum-level differences that affect clinical outcomes. Here’s what I learned that helps me appreciate why proton therapy offer distinct advantages:

Quantum-Level Differences
Proton Therapy:
• Protons are positively charged particles with mass. The cyclotron accelerates protons to an incredible speed of 125,000 miles per second. After leaving the cyclotron, their energies are adjusted precisely to control their penetration depth (Braggs peak) into tissue to reach the tumor but not beyond.
• Each proton carries a strong electromagnetic field, continuously interacting with electrons as it moves through tissue. Initially, the proton loses minimal energy. As it approaches the tumor site (the precisely determined depth), it rapidly slows down and experiences a rapid increase in interactions, depositing most of its energy within a concentrated region called the Bragg peak.
• Quantum mechanically, protons transfer energy via electromagnetic interactions (Coulomb interactions), energizing electrons in outer atomic orbitals, causing them to be ejected (ionization). These secondary electrons initiate additional chemical reactions and biological damage. After delivering its energy at the Bragg peak, the proton halts entirely, captures a nearby electron, and becomes a harmless hydrogen atom. This explains why proton therapy has essentially no exit dose, dramatically minimizing damage to healthy tissues beyond the tumor.

X-Ray (Photon) Therapy:
• X-rays are photons—massless, chargeless particles always traveling at the speed of light (~186,000 miles per second). Unlike protons, X-ray photon energies cannot be precisely adjusted to stop at specific depths.
• Quantum mechanically, X-rays interact discretely and probabilistically with electrons through processes like the photoelectric effect and Compton scattering. Each photon transfers energy to electrons, knocking them from their atomic orbitals. These electrons then further ionize atoms, causing widespread damage throughout their path.
• Due to their massless nature, X-rays can’t be slowed down or stopped by tissue. Consequently, they deposit energy continuously along their path, causing damage to healthy tissues both before and beyond the tumor.

Clinical Advantages of Proton Therapy
• Precise Depth Control: Proton pencil beam scanning leverages powerful electromagnets to shape and steer beams in precisely controlled spots (~5 mm diameter), delivered spot by spot, layer by layer (starting from the bottom going up) at thickness of around 3 mm. This allows exact targeting of tumor, significantly reducing exposure to healthy tissue.
• Precise Dose Control: Each treatment spot receives a dose that is adjusted dynamically, between 10–100 million protons, with billions distributed across the tumor for optimal efficacy while minimizing side effects.
• Minimal Exit Dose: The proton’s Bragg peak effect ensures minimal radiation beyond the targeted region, protecting surrounding healthy tissue from unnecessary damage and reducing the likelihood of side effects and complications.

DNA Damage Mechanisms
Both proton and X-ray therapies primarily kill cancer cells by causing DNA damage through ionization and the formation of highly reactive oxygen radicals, which break essential chemical bonds in DNA. However:
• X-rays: Primarily cause DNA damage indirectly via free radicals, making effectiveness highly dependent on oxygen presence.
• Protons: Produce substantial direct DNA damage in addition to indirect radical-mediated damage, especially concentrated at the Bragg peak. This direct, dense damage is more complex, making it significantly harder for cancer cells to repair and recover.

Biological Implications
Because proton therapy causes more complex and severe direct DNA damage, cancer cells find it much harder to repair themselves, enhancing treatment effectiveness. The reduced radiation exposure to surrounding tissues also improves patient quality of life by lowering the risk of side effects and complications.

By clearly understanding these detailed quantum and biological distinctions, I am confident that choosing proton beam therapy over X-ray therapy is the right choice for me.
I hope you will find this information useful in making your own decision. There are less than 50 proton centers in the US and traveling to and from creates a logistical problem for most patients including myself.

Coming face to face with Varian 360 Pro Beam on Day 1

This is a moment I will never forget during this entire journey: a three-story high, 190 - ton gantry assembly capable of rotating 360 degrees and aiming the proton beam at its target with 1 mm. accuracy. So precise and deadly to cancer cells yet so gentle to the surrounding tissues. It is a marvel of human thought and engineering. With the exceptional team managing my cancer care, I am confident that this is my best chance for cure with the fewest long term side effects. 

Here is a science fact to ponder and be amazed
The protons destroying the cancer cells and helping heal my body once roamed free right after the Big Bang. All protons were created about 13.8 billion years ago. The ones being used in my treatment are the same ancient protons that formed the first stars. 
Human created the room-sized cyclotron use for my treatment to accelerate protons to about 70% the speed of light. Nature creates cosmic rays which are high speed protons that constantly bombard our upper atmosphere. The most energetic proton ever detected was traveling at almost the speed of light that if it were to be make by human, it will require building a cyclotron the size of the our own Milky Way galaxy.

I am Dr. Rolando L. deGoma, a triple board-certified cardiologist with decades of experience in heart disease prevention. My journey into aggressive cardiovascular prevention began at age 55 when I discovered I have silent heart disease through coronary calcium scoring - an invisible but deadly condition that, if left untreated, could lead to a major heart attack or premature death.

Having never been hospitalized or undergone surgery (other than cataract extraction and colonoscopy), I now experience firsthand the uncertainties and decisions that come with a cancer diagnosis.

My Path - My Story

PSA test - only for screening, not for diagnosis

 My first elevated PSA was on June 15, 2017 and it was 6.6 with 26% free. The earliest PSA I can find was 1.1 in 2004. I am sure I had a few more before 2017. I was afraid of having a 12 - core blind needle in a urologist office using just local anesthesia, many of them were negative. I had no symptoms. Subsequently, they were 4.4 (after I took Cipro 500 mg BID for 10 days), 2.5 (after I started taking Finasteride 5 mg daily), 3.06, 3.48 and the latest one week before starting Proton Beam Treatment was 4.1 with 27% free. Start your annual PSA testing at around age 55.

I’m Dr. Ron deGoma, a preventive cardiologist, and in January 2025, I became one of the many men facing a prostate cancer diagnosis. I created Beat Your Prostate Cancer not just as a doctor, but as a patient — determined to share what I’ve learned and help others navigate their own journey with more clarity, support, and hope.

Why I Started This Platform
Like many of you, my journey began with a rising PSA. I was fortunate to have direct access to advanced testing myself— but even with a medical background, I quickly realized how difficult it is to get the right information at the right time. Too often, men are not being told about the full range of options available to them. Outdated screening methods or unnecessary treatments can lead to confusion, fear, and missed opportunities for early intervention.

What shocked me most was discovering that many diagnostic tests and scans considered standard of care in countries like the UK, Australia, and Singapore are underused in the United States — even when they’re already covered by insurance. That’s why I’m speaking up. Prostate cancer can be unpredictable - it can progress in different directions quietly without any symptoms. Get to it early - that's your best chance for cure. But once the horses start leaving the barn, they can go in many directions all at once. Then, it is a different game. Advanced stage 4 metastatic prostate cancer often requires triple therapy: androgen deprivation, chemotherapy and radiation (IMRT or Proton Beam).

I want every man to know that you can—and should—take an active role in your prostate health. The earlier you’re informed, the better your outcomes can be.

Lastly, Don't Have a Heart Attack
Most men with prostate cancer die not from prostate cancer but from something completely unrelated - a heart attack or stroke. The underlying cause of acute heart attack is sudden plaque rupture. Most men who are 50 and older have silent plaque build up in their coronary arteries and don't know it. There are no symptoms and they are not detectable by EKG and stress test. A quick, simple CT without IV contrast called coronary calcium scoring test can reveal their presence and an effective medical therapy by your physician can stop disease progression and prevent plaque rupture, thus stopping a heart attack before it happens. The same medical treatment that prevents heart attack, also prevents stroke. My coronary calcium score was 600 when I was 55 years old. I kept my LDL cholesterol around 40, LDL particle number <900, systolic blood pressure around 120, HgA1c around 6.5, weight around 135. All my plaques are stable and I am sure many of them have regressed. I have never experienced any cardiac symptoms. Ask your doctor about getting a calcium score test. You can get a calcium score without a prescription. Most insurance still refuse to pay for it. The cost of this potentially lifesaving test is between $70 to $100.

I have been aggressively preventing heart attacks since 2001 and in my practice, having a heart attack has become a rare event - most years - zero event rate. Heart attack remains the number one killer of adult Americans only because all health insurance companies including Medicare, unfortunately, only reimburse for heart attack care. There is no reimbursement for preventing heart attack in very high risk patients - even those with high calcium score.
